...authorize all medical and surgical treatment, X-ray, laboratory, anesthesia, and other medical and/or hospital procedures as may be performed or prescribed by the attending physician and/or paramedics during the course of the camp for your child(ren) and waive my right to informed consent of treatment. This waiver applies only in the event that neither parent/guardian can be reached in the case of an emergency.
...release Vance Trefethen, Training Minds Ministry, Monument Publishing and any other individuals from liability in case of accident during activities related to this event and non-debate related activities during the course of the event. You also understand that this camp has an open schedule that allows students to take responsibility of their own schedule. Unruly behavior of registrants is recognized as self-inflicted and is not the responsibility of Vance Trefethen, Training Minds Ministry or Monument Publishing.
...realize that 1) we have the right to terminate a student's participation, without refund of fees, if in our determination a student is disruptive to the program or engaging in conduct that unethical, illegal, or disrepectful to camp management, and 2) personal displays of romantic affection are prohibited unless the individuals doing so are married to each other and violation of this rule will be considered a disruptive behavior and will trigger the disciplinary termination mentioned above.

Furthermore, should your child(ren) go on to win or place in future speech and debate tournaments, you give permission to Vance Trefethen, Training Minds Ministry and Monument Publishing to use your child(ren)'s name for promotional material.
